The Connect2 Community Network has come a long way since we first developed our shared vision:

To strengthen the coordination of care for those in need by connecting people, service organizations, and community partners more quickly and more effectively – resulting in a healthier, more equitable community for all.

This website is newly launched and currently under construction. We have a lot of news and progress to share, as well as plenty of opportunities to learn more and join our community of 75+ organizations collaborating to build the network.
